 Keep well is a 2 year pilot project in north Glasgow for people aged 45-64 to help look after their health.

 9 GP Practices are participating in the project and if you are a patient of one of those practices and are aged 45-64 you will be invited for an appointment with your GP / Practice Nurse / Healthcare Assistant.

They will measure your height, blood pressure and ask you a few questions about your daily life. The appointment should take no longer than 45 minutes. You can ask to be involved in the project even if you have not yet been contacted by your surgery.

 

Additional support 

After the consultation a number of community services are able to offer advice and assistance in the following areas:

  • smoking cessation
  • stress relief / stress management
  • money/benefit advice
  • meaningful activity / employability
  • weight management
  • nutritional information
  • physical activity
  • alcohol
  • medication
  • literacy / numeracy
  • mental health.

You are under no obligation, but if you decide to use any of the services you will be fast tracked. All services are free and offered locally in north Glasgow.
